Children have been locked out of Casa Libre, the shelter, for hours forcing some to take shelter outside in a broken-down van, former residents said.

At the second one, officials discussed concerns with “financial issues, operational issues, physical plant issues, reporting requirements, care [and] supervision, administrator qualifications [and] availability, and staffing issues,” state licensing records show. Decision-making is largely decentralized and often left in the hands of regional offices and individual inspectors, she said.
