The panel found that three incidents in the run-up to Mrs McCabe's death indicated that Conor could potentially be violent
Police and medical professionals failed to spot the domestic violence warning signs in a man's behaviour before he killed his grandmother in a violent attack.
READ MORE:Petrified gran barricaded herself in room with sofa before devil-obsessed grandson beat her to death The review concluded that although Conor killing Mrs McCabe couldn't have been foreseen this was because agencies didn't recognise the various incidents as domestic abuse and therefore missed opportunities to identify and manage the risk the then 26-year-old posed to his granny.
There was no further intervention or involvement from mental health services with the review panel finding that the 24-minute assessment carried out by Ms Cox, who worked for Blackpool Teaching Hospitals NHS Foundation Trust, was inadequate. After Conor was arrested following Mrs McCabe's killing he was diagnosed with a severe form of paranoid schizophrenia and sentenced to indefinite detention in a medium-secure hospital.
Referring to two events in December 2017, when Conor "went berserk" after his aunt threw out his cannabis stash and another in April 2019 when the sexual incident took place, the report's author said: "Nobody recognised that these incidents amounted to domestic abuse and therefore policies and procedures were not followed."
