after pharmaceutical giant Gilead Sciences altered its reimbursement policy for the lifesaving HIV prevention drugs known as PrEP.This is how the policy used to work: Pharmacies would dispense PrEP to certain eligible patients at a discounted price, but Gilead would reimburse at the retail cost of the drug. The difference, or “spread,” sometimes up to $1,200 per patient, would get passed along to the clinic, where it was often used for wrap-around services like HIV testing.

“Gilead is sympathetic. They’re not in denial of what their effect has been. But they’ve been very clear that they’re not responsible for HIV safety net services in this country,” Carlo said.in February. In response, its clinics began charging patients on an income-based sliding scale.Prism Health will also be shifting away from HIV-only services. The nonprofit is applying to become a federally qualified health center.

The downside, said Carlo, is that Prism Health cannot survive if it doesn’t expand its services beyond HIV and STIs. After this period of retraction, he hopes the organization will grow and, if it gets federal approval, continue to serve its HIV patients — and anyone else in the community who may need their help.
